EDS WellBeing collects and holds personal information about you for the purpose of providing health and wellbeing services.

What information we collect:
We collect personal information including your name, contact information, address, and health and medical information relevant to your care.

How we collect your information:
We collect information directly from you — for example, through intake forms, during consultations, or via our online booking system.

We also collect information indirectly from third parties. This includes:

  • Referrals and clinical notes from GPs, specialists, or other health providers
  • ACC claim and injury information (collected as authorised under the Accident Compensation Act 2001)
  • Information shared as part of co-managed or shared care arrangements
  • Information provided by a family member, caregiver, or authorised representative on your behalf

When we collect information about you from a third party, we will notify you as soon as reasonably practicable.
We will tell you what information was received, where it came from, why it is being used, and how you can access or correct it.

We collect your information in order to:

  • Assess, plan, and provide appropriate support and care
  • Communicate with you about your care and follow-up
  • Meet our obligations under the Accident Compensation Act 2001 where an ACC claim is involved

Who we may share your information with:

  • Other healthcare providers involved in your care
  • ACC, where an ACC claim has been lodged, as required under the Accident Compensation Act 2001
  • Trusted service providers who assist with storing or managing our systems, under appropriate confidentiality arrangements

Providing your information is partly optional. If you choose not to provide relevant health information, we may be unable to provide appropriate support.

How we keep your information safe:
We store your information in secure, encrypted systems and limit access to authorised staff only.

How long we keep your information:
We retain your information for a minimum of seven years, after which it is securely destroyed or permanently deleted.

Privacy breaches:
If a privacy breach occurs that is likely to cause serious harm, we will notify you and the Privacy Commissioner as required under the Privacy Act 2020.

Your rights:
You have the right to ask for a copy of any personal information we hold about you, and to request a correction if you believe it is inaccurate.
